1. Purpose of Data Collection
By submitting this form, you consent to Dream Planners Group and Great Eastern Financial Advisers Pte Ltd ("GEFA") and its authorised representatives collecting, using, and disclosing your personal data (name, phone number, email address, and profile information) for the purpose of scheduling and conducting your complimentary financial health review, and contacting you about financial products and services distributed by GEFA through all channels including SMS, phone call, email, and social media.
2. Data Protection (PDPA)
Your personal data is collected in compliance with the Singapore Personal Data Protection Act 2012 (PDPA). Your data will not be sold to third parties. It may be shared with licensed financial consultants within GEFA solely for the purpose of conducting your review. Data will be retained for no longer than 12 months or until you withdraw consent, whichever is earlier. 3. Nature of the Review
The financial health review is informational only and does not constitute financial advice under the Financial Advisers Act (Cap. 110). You are under no obligation to purchase any product or service. This service is available to Singapore residents aged 18 and above.

Dream Planners Group is part of Great Eastern Financial Advisers Pte Ltd ("GEFA"). By submitting this form, you confirm that you have read and consent to GEFA and its authorised representatives collecting, using, and disclosing your personal data in accordance with the
Great Eastern Life Data Privacy Notice. Your consent does not supersede any previous consents you may have given to GEFA.
